당뇨환자에 뜻밖 희소식?…혈당관리 잘하면 알츠하이머 치매위험 낮아

우리나라 사람들의 식단도 점차 서구화 되어가고 있습니다. 그러다보니 당뇨병 환자도 빠르게 증가하고 있습니다. 이로인한 사회적 비용도 엄청나게 늘고 있고 개인적 고통도 이루 말할 수 없을만큼 큽니다. 하지만, 관리하기에 따라 얼마든지 정상생활을 할수가 있습니다. 당뇨환자도 혈당관리를 잘하면 알츠하이머 치매위험을 낮출 수 있다는 연구결과도 발표됐습니다. 

 


빠르게 증가하는 당뇨병 환자 어떡해

우리나라 사람들의 식단이 서구화 되면서 고혈압과 함께 국내 당뇨병 환자도 빠르게 늘어나고 있습니다. 당뇨병이 무서운 것은 그 합병증 때문입니다. 우리 몸은 음식을 소화라는 과정을 거쳐 핏속의 혈당으로 전환돼 세포 에너지원으로 쓰이게 됩니다. 하지만 과잉섭취로 혈액 속에 남아 있는 고(高)혈당은 일종의 가시돌기 역할을 합니다.

 

이런 당뇨병으로 인해 알츠하이머 치매 발생 위험도 높인다고 합니다. 치매의 40%는 동맥경화로 발생하는 혈관성 치매인데, 당뇨가 있으면 뇌혈관 동맥경화가 생겨 치매 발생 위험이 커진다고 합니다. 이쯤되면 당뇨병을 쉽게 볼 수 없는 이유이기도 합니다.


2형 당뇨병 환자 혈당관리 잘하면 알츠하이머 치매 위험 낮출 수 있다?

2형(성인) 당뇨병 환자는 혈당관리만 잘하면 알츠하이머 치매 위험을 낮출 수 있다는 연구결과가 발표돼 눈길을 끌고 있습니다(Keeping blood sugar levels in check may have a protective effect against dementia in people who have type 2 diabetes, new Swedish research suggests).

 

스웨덴 예테보리(Gothenburg) 대학 의학연구소의 아이딘 라우샤니 박사의 연구결과 당뇨병 환자라 하더라도 혈당관리만 잘하면 알츠하이머 치매 위험을 낮출 수가 있다고 합니다("The positive association between [average blood sugar levels] and risk of dementia in fairly young patients with type 2 diabetes indicates a potential for prevention of dementia with improved blood sugar control," study author Dr. Aidin Rawshani, from the National Diabetes Register and Institute of Medicine in Gothenburg, Sweden, and colleagues wrote).

'당뇨병 환자 혈당관리 잘하면 알츠하이머 치매 위험 낮출 수 있다' 어떻게 연구?

스웨덴 예테보리(Gothenburg) 대학 의학연구소의 아이딘 라우샤니 박사는 당뇨병 환자 35만명(평균연령 67세)의 8년간(2004~2012) 의료기록을 조사분석 했더니 당뇨병 환자라도 혈당관리만 잘하면 알츠하이머 치매 위험을 낮출 수 있다는 사실을 알아냈다고 합니다(The study included almost 350,000 people with type 2 diabetes. They were all registered in the Swedish National Diabetes Registry between January 2004 and December 2012. They had no history of dementia when they were diagnosed with type 2 diabetes. The mean age was 67 years when the study began). 





혈당 수치 높으면 낮은 환자보다 치매 발생률이 50% 가량 높게 나타나
스웨덴 예테보리(Gothenburg) 대학 의학연구소의 아이딘 라우샤니 박사의 연구 결과 2~3개월간의 장기혈당을 나타내는 당화혈색소(A1c) 수치가 10.5% 이상인 당뇨병 환자는 6.5% 이하인 환자에 비해 치매 발생률이 50% 높은 것으로 나타났다고 합니다(After taking other variables into account, the study found that those with HbA1c levels of 10.5 percent or higher were 50 percent more likely to be diagnosed with dementia compared to people with HbA1c levels of 6.5 percent or less).


이 결과는 환자의 연령, 성별, 당뇨병 진행기간, 흡연, 혈압, 체중, 신장기능, 당뇨약의 종류, 부정맥 등 치매와 관련된 위험요인들을 고려한 것이라고 합니다.


미국 당뇨병학회 당뇨병 환자 혈색소 수치 7% 이하로 관리 권장
미국 당뇨병학회(ADA)는 당뇨병 환자의 경우 당화혈색소 수치를 7% 이하로 관리하도록 권장하고 있습니다(Study volunteers were tracked until the study ended in 2012 or they were hospitalized for dementia or died. Using a computer model, the researchers calculated the link between average blood sugar levels and dementia. Average blood sugar levels were based on the results of hemoglobin A1C tests (HbA1C). This test provides doctors with a several-month average of blood sugar levels, according to the American Diabetes Association (ADA). The ADA generally recommends an HbA1C level of 7 percent or less for people with diabetes).


흔히, 당뇨병 환자는 일반인에 비해 치매 위험이 높은 것으로 알려져 있습니다. 그렇기는 하지만 혈당, 특히 장기혈당 관리가 치매 위험에 어떤 영향을 미치는지에 대한 연구결과는 아직 없었다고 합니다(Evidence is growing that diabetes increases the risk of future declines in brain function. 당화혈색소가 뭐기에

우리가 알고있는 적혈구에는 혈색소(헤모글로빈)라고 하는 중요 단백질이 들어있습니다. 산소를 운반하는 적혈구의 혈색소(헤모글로빈) 분자가 혈액 속의 포도당과 결합한 것입니다. 혈중 포도당은 혈색소의 일부와 결합하게 되는데 이렇게 혈색소에 포도당이 결합된 형태를 당화혈색소라고 부릅니다. 정상인의 당화혈색소 범위는 4~6%이고 당뇨인의 당화혈색소 조절 목표는 6.5~7% 이하로 유지합니다. 당화혈색소가 1% 올라갈 때마다 혈당치가 평균 30㎎/dL 정도 올라간다고 합니다.

 


 

뇌졸중 겪은 당뇨병 환자 치매 위험이 40% 높아

스웨덴 예테보리(Gothenburg) 대학 의학연구소의 아이딘 라우샤니 박사의 조사분석 결과 예전에 뇌졸중을 겪은 당뇨병 환자는 뇌졸중 병력이 없는 환자에 비해 치매 위험이 40% 높은 것으로 나타났다고 합니다(In a cohort of 349,299 patients with mean age of 67 years at baseline, 11,035 patients (3.2%) were admitted to hospital with a primary or secondary diagnosis of dementia during a mean follow-up of 4.6 years. Patients with a HbA1c of 10.5% or higher (worst blood sugar control) were 50% more likely to be diagnosed with dementia compared to those with HbA1c of 6.5% or less (most well controlled). Previous stroke in these patients made them 40% more likely to develop dementia than those without stroke).
